Fire Incident Report Form

Just like other safety events, your team should report and document all fire incidents, especially those that result in property damage or injuries. This basic report form outlines the information you should include in your report, including:

  • Outcomes of the fire (injuries, property damages, etc.)
  • Incident details (fire department involvement, reporter information, etc.)
  • Possible sources/causes of the fire
  • Incident description and supporting attachments

Like any template, you can customize the information your employees must provide in this form so that it’s more site-specific. That way, you can gather certain data points to reference in your site’s EHS reports.
