Perform a 5 why root cause analysis with Frontline ACT

5 why analysis with no action items might as well be called a 5 why paralysis. 

Make sure your team follows through on incident investigations by having them document their findings in the Frontline action tracking system. 

After filling out a 5 why analysis form, your team can schedule and assign corrective action items or add notes for greater context. 

That way, you can keep moving forward and avoid the paralysis that happens when you find a root cause but fail to implement new control measures.
