Forklift Operator Evaluation Form

OSHA standard 1910.178 covers the safe use of powered industrial trucks (PIT), which include forklift equipment. You must regularly assess all forklift drivers at your facility to ensure compliance with these regulations. The evaluation should look at each stage of the driving process:

• Pre-operation checks
• Picking up a load
• Traveling with a load
• Setting down a load
• Parking
• Fueling and battery recharging
