Check Out Equipment Form
An organized and clean operation is a safe one. Part of keeping an organized workspace is having a system to store and keep track of critical equipment.
Whether that be personal protective equipment (PPE), IT assets, tools, etc., it’s important that you know where everything is and who has it.
This simple form for checking out equipment helps you document when someone checked something out and checked it back in. That way, if an asset goes missing, you should have some idea of who had it last.
If you don’t follow the checking in/out system consistently, however, this form won’t help you organize your workspace. You must ensure that all workers follow the process, so you have all the information you need to track down missing equipment.
