Hazardous Materials Disclosure Form

Notifying local responders of the hazardous materials you have in your facility is essential to any emergency response plan. This disclosure form template includes several key pieces of information:

• Contact information for the facility/building managers
• Contact information for the cleanup service provider
• Various uses of hazmat materials in your facility
• Inventory log of all the materials in your facility

The purpose of this document is to communicate ahead of time what types of hazards are present at a site. That way, if a major hazmat incident occurs, emergency responders will already have insight into what types of risks to anticipate.
