Completing a hazard and operability (HAZOP) study can help you foresee areas within a system that require additional control measures. It can be overwhelming, however, because there are quite a few steps to follow.

This is an Excel template that contains three tabs. The first is a template document which includes all the elements listed above. The second tab contains instructions for filling out each area of the template. And the last tab includes definitions of each element, as well as, the guide words your team should use.
