Personal Protective Equipment Form

The purpose of this Word template is to break down a job task into the various hazards that may require PPE.

Each section covers a different area of the body and provides some of the most common hazards associated with it. You can add or remove line items for the hazards you want to review.

For each hazard, state whether there’s required PPE involved and list out the requirements as well. Then, fill out the action items you may need to finish so that workers have the PPE you need.

This may involve ordering more supplies, installing PPE cabinets, creating new 5S spaces for communal PPE, and more. Go through this process for each of the job tasks where workers may need protective equipment to stay safe.
