Review Level Form

The goal of this review level form is to help you figure out which type of process change you’re making and who needs to approve it. Your changes can fit into one of three categories:

LEVEL 1: Field supervisor approval required

LEVEL 2: Area supervisor approval required

LEVEL 3: Area manager approval required

After answering the series of yes or no questions on this form, you should be able to categorize your modifications into one of the three levels. The questions cover a wide range of topics, including things like the impact on workplace equipment, system controls, and process inputs. From here, you should know how your changes might impact worker safety and who to notify.
